US Passports

Submit your passport application at the Hodgkins Library

 

We do not accept, or recommend, courier/hand carry passports. These services will usually cost the traveler more and doesn’t save much time.

Submit your U.S. passport application at the Hodgkins Library! The Hodgkins Public Library District is an official designated Passport Acceptance Facility by the U.S. Department of State. For more information go to the Department of State’s website at www.travel.state.gov. To schedule an appointment, call the library at 708-579-1844.

Payment

  • U.S. Department of State fees (Payable to the U.S. Department of State), check or money order
    • Adults (16 yrs. & over): $130 (Book), $30 (Card)
    • Minors (15 yrs. & under): $100 (Book), $15 (Card)
    • Optional: Expedite fee: $60 extra for each application, added to U.S. Department fee check
    • Optional: Overnight Delivery fee: $21.36 extra for each application, added to U.S. Department fee check
  • Library Execution fee, for each application (Payable to the Hodgkins Public Library)
    • $35, cash, check or money order

What to Bring

Adults

  • Adults who are applying must be present
  • Original birth certificate, naturalization certificate, etc. & previous passport (if applicable)
  • Passport size pictures
  • Valid ID (State ID, Driver’s license, Military ID, Passport (U.S. or Foreign)

16 and 17 year olds

  • One parent listed on birth certificate must be present
  • Original birth certificate, naturalization certificate, etc & previous passport (if applicable)
  • Passport size pictures
  • Valid ID (State ID, Driver’s license) and Parent Valid ID (State ID, Driver’s license, Military ID, Passport (U.S. or Foreign), Matricula, etc.)

Minors (15 yrs. old & under)

  • Parent(s) listed on birth certificate must be present
  • Children who are applying must be present
  • Original birth certificate, naturalization certificate, etc. & previous passport (if applicable)
  • Passport size pictures
  • Parent(s) Valid ID’s (State ID, Driver’s license, Military ID, Passport (U.S. or Foreign), Matricula, etc.)
